Custom Pleating Order Forms

Before placing a custom pleating order, please first read "How to Order Pleated Fabrics." If interested in sunburst pleating, please read "How to Order Sunburst Pleating."

To place a custom pleating order, download the correct form for the style of pleating you need and fill in all required information. A separate form must be filled out for each pleat style and fabric type in your order. If you are shipping your fabric to us, all forms must be included in the package to avoid any potential delays.

This breaks down all potential trouble sources when sending pre-cut or pre-hemmed fabric for sunburst pleating. We must receive this checklist initialed and signed along with your order form and fabric.
